Dental Benefits

The MetLife Dental program provides eligible employees access to a national network of credentialed general and specialty dentists. Using an in-network provider gives you the maximum dental benefit. Visit metlife.com/dental for a list of participating providers. Your dental network is PDP Plus Network. Please note: You won’t receive a Dental ID card. Let your provider know you’re covered by MetLife, and supply your Social Security number and the group number: 229496

Type D – Orthodontia: Orthodontic Treatment (children to age 19), Orthodontic Appliances	Plan pays 50%
Orthodontia Lifetime Maximum*	$2,000

*Dependents and adults enrolled in the dental plan are eligible for orthodontia benefits up to the lifetime maximum.
